What Documents do I Need to Apply for a Position with New Care?
- Identity and Employment Eligibility Documents
- TB test results: you must provide either a negative result documented within the past 12 months or a positive result and subsequent chest x-ray that shows you are free and clear of TB. If you do not have a result documented within the past 12 months, you must have a two-step TB test. A two-step test requires that the first PPD result be read between 48 and 72 hours of the injection. A second PPD must be performed within two weeks of this first step. Both results must be submitted.
- Name/address/phone number of two professional/clinical references
- Professional License
- Proof of a Hepatitis B vaccination: you must provide documentation of the three injections or a positive titer. Employees who have not had the vaccination will be provided the opportunity to have the vaccination. New Care will pay for this vaccination. If you have had the vaccination, but do not have proof, or if you do not want the vaccination, you may sign a waiver.
- Documentation of any certifications
- CPR Card (includes adult, child and infant skills)
